Included complimentary services must be used within the applicable monthly billing cycle and do not roll over to future months.
If the Member chooses not to use their included complimentary monthly service during the billing cycle, the monthly membership payment may be applied as a credit toward another qualifying service performed during that same billing cycle.
Membership credits may not be accumulated, transferred, redeemed for cash, or carried forward to future billing periods.

If the Membership is canceled prior to completion of the initial 6-month term, the patient agrees to pay the difference between the total membership payments made, and the standard retail pricing of all services, treatments, lab work, provider visits, discounts, and products received during the membership period.
Any outstanding balance resulting from early cancellation will be charged to the payment method on file and must be paid in full prior to termination of the Membership.
